Given his age, he likely won't last long. The surgery would be risky at his age. 1poorMIL has kidney cancer, but hers is very slow. The urologist says she'll likely die of old age before the cancer gets her, so it isn't worth risking surgery. With aggressive cancer, Biden probably has to risk the surgery, but it will be hard on him.

According to the NYT, Biden’s cancer is hormone-sensitive “characterized by a Gleason score of 9 (Grade Group 5) with metastasis to the bone.” Surgery would be contraindicated. More than likely, he will have triplet therapy: Androgen deprivation therapy (ADT), Chemotherapy (docetaxel), and an Androgen receptor pathway inhibitor (ARPI) plus zoledronic acid to promote bone health.
Even at his age, he could easily live several more years with this treatment plan.

Her “Doctorate” is in education. The word “Doctor” doesn’t just apply to people who say “Open your mouth and say ‘Ahhh’”. Martin Luther King Jr had a Doctorate (Systemic Theology), as does Brian May, guitarist for Queen (Astrophysics). Also Shaq O’Neill (Education) and Rachael Maddow (Philosophy).

How could Biden's cancer have reached stage 4? The CDC does not recommend prostate cancer screening for men over 70. It can lead to overtreatment, since most men who live long enough die with prostate cancer, not of prostate cancer. In Biden's case it looks like screening earlier would have helped.
I would have expected them to run a PSA test as part of his regular blood work. PSA not super reliable, though, as I know from personal experience.
