💡LISTEN UP—I would start STOCKPILING these flu medicines (xofluza, relenza, Tamiflu) for your family STARTING NOW. If bird flu is ever human-to-human transmissible… you will regret it when near-certain shortage hits. (Many epidemiologists/MD friends all agree & already started)

2) Yes these are all prescription flu medicines. You need a prescription. I’m saying next time you get flu symptoms of any sort, ask for it. I’m just sharing what many epidemiology and medical colleagues are already doing. Don’t shoot the messenger.

3) I’m aware this pre-stockpiling of prescription medicines is an extremely privileged thing where not everyone can do it. And you should only take it if you have the flu, but it doesn’t hurt to ask your doctor if they can dispense it if they deem appropriate. Others agree.

4) And yes, these 3 prescription flu drugs are all deemed as likely efficacious against (most) H5N1 bird flu strains. Other experts agree.

8) Sidenote: I HAVE ZERO CONFLICTS. None of the flu medicine companies pay me, none have contacted me or asked me for any promotion, and no COVID/flu tests companies pay me either. This is purely an FYI of what many HCWs/scientists are already doing. Don’t shoot the messenger.

9) Another anecdote I’ll share—because children under 12 years old are not eligible technically for Paxlovid if COVID positive, some doctors and nurse practitioners have been prescribing Tamiflu for kids. There is some data showing faster viral clearance

10) Scientists are not trying to fear monger. The fact that ferrets had 100% mortality to bird flu, and ferrets are one of the best animal models for human infection, is very worrisome.

11) TO BE CLEAR—absolutely do not lie to your doctor. You can actually get Tamiflu as a prophylactic drug — ➡️you can get the drug for PREVENTION of infection, before you get sick. And does it work for flu prevention? YES it does. So ask your doctor.

12) FDA approved Tamiflu for PREVENTION too— Folks, the FDA even grants approval for both treating and preventing flu. “Additionally, oseltamivir is considered suitable for prophylactic use in patients 1 year and older.”

The problem with that is the medicine expires, sometimes fairly quickly. You'd have to show up with symptoms every six(??) months to get a new script.

But, yeah, I get what he's saying.

Ya know, the Army did a study on the expiration dates and found the medicines were effective many years after the expiration dates. And none of them degraded and became something poisonous. So I'd have no problem taking Tamiflu a couple of years after the expiration dates. I'm using antibiotic ointments many years after the expiration date and they're still effective. I've used up pills after the expiration date and they still cleared my sinuses, and stopped infections. That's just me though. But - worth a read

SNIP Solid dosage forms, such as tablets and capsules, appear to be most stable past their expiration date. Drugs that exist in solution or as a reconstituted suspension, and that require refrigeration (such as amoxicillin suspension), may not have the required potency if used when outdated. Loss of potency can be a major health concern, especially when treating an infection with an antibiotic. In addition, antibiotic resistance may occur with sub-potent medications. Drugs that exist in solution, especially injectable drugs, should be discarded if the product forms a precipitant or looks cloudy or discolored. SNIP

Expats would buy antibiotics for Chickens at the agrivet, try and figure the dosage and take that. I used to get amoxicillin before a trip - helped one time because I got some bad food and the doc didn't prescribe. I took the complete round because my symptoms were still there, knocked it right out.

Agree, with a caveat or two

It depends on large part on the storage conditions. Meds in a first-aid kit in the trunk of my car (epi-pens, Narcan) I try to replace more-or-less as they expire, i.e. within a year or two of the expiration date. It gets hot in there.

Most of my backup meds, however, are kept in the dark, specifically in a military surplus ammo can -- the type with a waterproof seal -- in the basement. There they are in the dark in a perpetual 50-70 degrees or so with controlled humidity. Those meds I anticipate to stay at or near their original potency more or less forever.

(There are a couple of exceptions, as the active molecules spontaneously degrade over time. Sublingual nitroglycerin (unstable angina) is one. For some reason, tetracycline (older antibiotic) and possibly dilantin (seizures) stick in my head from my med school pharmacy course of, uh, 40+ years ago as another two. I'm not sure of this last statement, however. Regardless, these aren't meds I keep in the kit).

IIRC, the study said pills and tablets stayed effective long after the expiration dates. Solutions dropped in effectiveness quicker. Nothing on ointments, but I've found antibiotic ointments, etc., effective years after the expiration date. Some weren't and I had to get new.

But there was a rumor going around Asia about one antibiotic, that if it was old it would blow your kidneys out. They did a study on that and concluded it was likely a myth.
