MD's who "retreat" to a single $ generating procedure make a splendid return
It depends on the specialty.
I had an orthopedic surgeon who only did hip and knee replacements. VERY good at it. Each patient required significant time in the OR and some post-surgery care. He did three or four surgeries per day, two days per week. Booked the OR for 12+ straight hours, one day/week (different days) at two different hospitals. So, there was a strictly limited number of patients he could actually treat each year.
He DID reject doing surgery on patients who were NOT "fully prepared" on the day of the scheduled surgery. I got moved up 3-4+ hours for my surgery for that reason. He deliberately had one or two extra patients ready for surgery in case a scheduled patient was NOT ready (for *any* reason).
